I have completed the achievement on 104 retail titles, 20 arcade titles and 27 pieces of DLC and have unlocked numerous achievements on dozens if not hundreds of other games. There is absolutely no way in hell I could ever remember every single achievement I've ever unlocked or how hard it was.

I think he/she/it means achievement you've actively gone after and found hard, not just the random ones you get. I don't aim to get them but I did see the dead rising 1 achievement where you had to play constantly for something like 14 hours which seems more like a chore than anything.

Think there was one for bad company 2, getting a road kill in a helicopter. It was on purpose and I wasn't crashing...honest... :oops:

Probably completing Dishonored without killing a single person. However, the achievement didn't actually unlock when I KNOW I didn't kill anyone :(

I was more than pissed off when that happened.

X-Com - Classic IronMan - according to TrueAcheivements anyway.
I used to make arrow in the knee jokes , then Arm Fall Off Boy kicked me in the balls....... Come on Boon , quit teasing us with DLC characters we don't want , give us Arm Fall Off Boy dammit! Or at least give us Detective Chimp........
